I am not sure if your old game saves will work with the GOG version if they were made by the Steam version. You can call it a fix if you will.When you make a new game and then it crashes, then watch thi. From my experience it seems like the *.ini's made by the Steam version do not work well with the GOG version. This is a tutorial on how to make Fallout 3 work on Windows 7. Once you are done with that, hit PLAY and start a new game. What I would suggest is make a backup of your old games saves then delete the Fallout3 folder and start the game via FalloutLauncher.exe and setup all your graphics options. (PS: I updated my video drivers few weeks ago so this should be current as well) As far as I know, d3d9.dll is only needed if you are using Intel graphics. I ve tried to follow the guides about installing different direct x (does nothing), putting the game to compatibility mode as XP SP3, Vista or 7 (does nothing), running as admin (does nothing), downloading d3d9.dll and placing it in the game's folder (immediate crash after the clicking "Play" in the launcher, so it even worse), editing config file (still crashing). I am having the same issue with Fallout New Vegas (except it crashes right after playing the intro where we are introduced to the faction and getting shot). When I launch the game it works good straight out the box, however when I try to run a new game or load a save from the game I played 10 years ago it crashes. Few days ago I decided to refresh the memories and purchased the GOG version.
